It would basically make things that give/take red hearts (including pickups and sacrifice rooms) act like they give/take soul hearts instead (functionally, not actually), and vice versa - at the cost of reduced chance for devil/angel deals. Soul heart trades still cost 3 Soul hearts. To get the Adult transformation you need to take three of the Puberty pills in one run. Scapular - Adds 1 soul heart when the player is downed to 1/2 a regular heart. If you have only 1HP left the pill will act as a Full Health.
